The GPD Case Management Grant (Opportunity Number: VA GPD CM FY2026; CFDA 64.024) is a Department of Veterans Affairs Homeless Providers Grant and Per Diem (GPD) renewal funding opportunity designed to keep vulnerable Veterans stably housed. The program focuses on supporting Veterans who are at risk of becoming homeless, as well as Veterans who were previously homeless and are moving into permanent housing after participating in transitional programs such as the GPD program or the VA Health Care for Homeless Veterans (HCHV) Contracted Residential Services (CRS) program. The core purpose of the grant is to fund case management services that strengthen housing retention, helping Veterans maintain stability once they are in housing rather than cycling back into homelessness. This is a renewal-only competition with extremely limited eligibility. Only the 90 current GPD case management grantees are allowed to apply, specifically those that already hold a GPD case management award with a start date of October 1, 2023, issued under the FY 2024 Case Management NOFO (VA-GPD-CM-FY2024) that was posted on Grants.gov on February 3, 2023. No new organizations or other applicants can apply, even if they otherwise fit common eligible applicant categories (for example, certain units of government, tribal governments, public housing authorities, and 501(c)(3) nonprofits). In practice, eligibility is restricted to the currently active cohort of 90 grantees listed on the GPD website.

Funding is expected to be significant and multi-year. VA anticipates approximately $15 million per year for three years to support these renewals, with awards expected to run for a three-year project period beginning on or around October 1, 2025. Individual annual award amounts are projected to range from about $75,000 to $300,000 per year, largely depending on the number of case manager positions being supported. Across the portfolio, VA expects to make roughly 90 renewal awards supporting approximately 120 full-time equivalent (FTE) case manager positions. A key cost constraint is that total costs may not exceed $150,000 per FTE case manager position per year. Applicants may request the number of case managers needed for their proposed project, but they must request at least 0.5 FTE (at least 20 hours per week on average) and they may not request more than the number of FTE positions approved under their current grant that began October 1, 2023. Applications also need to clearly justify and document the need for the requested staffing level.

The deadline is firm: applications must be received no later than 4:00 p.m. Eastern Time on February 26, 2025. VA states that late applications will be deemed ineligible, and it encourages early submission to avoid last-minute technical or administrative problems. To submit on time, applicants must have an IRS Entity Identification Number (EIN), a Unique Entity Identifier (UEI), and an active System for Award Management (SAM) registration.

Submission is electronic only and must follow the instructions on the GPD website. Applications cannot be submitted by email, mail, fax, or any other method. VA also emphasizes that the application must be received as one complete package; documents or components submitted separately will not be added later and may cause the application to be rejected.
